About Us

Gippsland Southern Health Service (GSHS) is a major provider of healthcare in the South Gippsland Shire. GSHS offers a broad range of services that meet the needs of a diverse and sparsely populated area. With two main acute facilities based at Leongatha and Korumburra, GSHS offers a broad range of specialist, general, acute, subacute, ambulatory, residential aged care and community services.

 

About the Role

We are seeking a suitably experienced individual to work in our Gateway program at GSHS.  The Gateway program is the point of entry for all Allied Health and Community Services referrals. 

 

About You

The successful applicant will have previous experience in assessment or similar role within the health and/or community sector with a thorough understanding of health and social issues that can affect clients.  You will also possess excellent communication skills and be able to work independently with minimal supervision.

 

Salary Packaging is available for all staff.  The successful applicant will be required to provide a current satisfactory Police Check, Working With Children Check and NDIS Worker Screening Check.

Selection Criteria
  • Essential : I have had at least my first dose of the COVID-19 vaccine, or if I am successful in obtaining this role, I am willing to receive a COVID-19 vaccine before commencing.
  • Essential : Previous experience in assessment or similar role, within the health and/or community sector.
  • Essential : Minimum Certificate IV qualifications in community health related discipline.
  • Essential : A thorough understanding of health and social issues that can affect the client and a demonstrated understanding of utilising resources and services available to meet client health needs/goals.
  • Essential : Demonstrated skills and experience in service coordination and the ability to undertake an Initial Needs Identification utilising a solution focussed approach.
  • Essential : Demonstrated understanding of risk assessment and prioritisation frameworks and the ability to apply these principles in practice.
  • Essential : Demonstrated capacity to prioritise and manage a constant workload whilst meeting program timeframes.
  • Essential : Demonstrated computer literacy skills including the ability to operate in MS Windows environment, database management and electronic information exchange, including telephone-based client management.
  • Essential : High level communication skills at both an individual and group level with an ability to establish rapport across a diverse range of stakeholders.
  • Essential : Well-developed teamwork skills and the ability to work independently with minimal supervision.
  • Desirable : A good understanding and awareness of the Active Service Model and Diversity.
